All on Four Dental Implants

"Teeth in a day" concept

This is not your grandparents denture. The All on Four is a treatment concept where patients suffering from a failing dentition can have all their teeth removed, four implants placed (per arch), and a permanently "Fixed" prosthesis made all in one appointment under IV Sedation. Our patients will walk out of the office with teeth that will not come out unless removed by the dental team at Texas Denture Clinic. This prosthesis has no palate, no excess pink flange, or material.

What is so great about the All on Four?

It is a graft-less solution. There is no bone grafting required. In the late 1990's patients wanting a fully fixed implant treatment plan would have had to under go a massive procedure in the operating room where a hip graft would have been done and carried the bone into the maxilary sinuses for a double sinus lift. Wait 6 months, then place 8 implants (vertical), then wait 4-5 months and then make the final fixed prosthesis. This price tag was 100,000.00 per arch and took over 1 year to finalize. Currently the All on Four is 20% of that 100,000 price.

Today we angle the posterior implants so no sinus lifting or bone grafting is needed. This is an xray of one of our recent cases, note the tilted posterior implants.

The prosthesis will be screwed down at the time of surgery, pending sufficient torque achieved on insertion of the implants. There will be no palate, limited pink, and no worry about having to remove the bridge.

This is a one surgery process. There is no second or third surgery required.

What are the steps involved?

First, an initial consultation with Dr. Damon is required. We will take our own CT so we can plan your surgery in 3-D. We will go over your medical history, evaluate your facial profile, your desires with your new teeth, and evaluate your anatomical structures. After meeting with Dr. Damon, the treatment plan will be gone over with our treatment and implant coordinators. Once treatment has been accepted by you, we will start with impressions - molds of your teeth so our lab can create your temporary prosthesis.

You will then be brought back for a second wax viewing appointment so you will see the teeth before surgery. You can make changes to them at that time and we will go over more per-operative information. Then we will schedule your surgery.

Surgery will include IV Sedation, tooth extractions as necessary, 4 implants specially angled to avoid structures, 4 abutments, and the temporary "bridge". This bridge or fixed implant denture, will have minimal pink acrylic, it will be palate-less, and screwed onto the implants. Please note that MOST of the time we can screw the bridge onto the implants, but on occasion the bone quality can be so poor that we will deliver a removable denture and convert to a fixed bridge after 4-5 months.

Success of the Procedure?

In our hands, implant "integration" success is around 97%, the superstructure or "bridge" success is about 98% successful over a 10 year period. This procedure was developed in around 1993, with mainstream acceptance by dentists and patients in early-mid 2000's.
